Athletic Paperwork Submission
The Lexington ISD Athletic department has decided to go to an electronic delivery system for school physicals, and medical history. All of these forms are REQUIRED before an athlete can participate in any athletic activities including practices. In order to access this online system go to and follow the online prompts available by clicking the red “Parents Click Here” prompt located at the top of the screen.

This policy for physicals is set by the UIL.
If your student participated in an extracurricular activity that requires a physical in 2019-2020, and turned in that physical—they DO NOT NEED A NEW PHYSICAL for 2020-2021.

You are required to fill out a NEW Medical History form. If you can answer YES to any of the first six (6) questions, then your student will need a NEW PHYSICAL.

If your student DID NOT participate in an extra-curricular activity that requires a physical in 2019-2020, they DO NEED A NEW PHYSICAL for 2020-2021.

All incoming 7th graders will need a physical.

If your student is NEW to the district. They will follow the policy stated above. You MUST turn in a copy of the 2019-2020 PHYSICAL to the Head Athletic Trainer.

LMS Boys Workout Information:
Beginning on Monday, June 15th, the Junior High boy’s workout time slot will be 8 a.m. to 10 a.m. This will ensure your son’s safety and social distancing practices that the state is requiring, as well as allow us to better coach your son during the sport specific part of their workout. If you have any questions feel free to contact any of the boys’ coaches.
